In order to begin work for you, we must receive an Application for Services.

An application can be obtained from one of our offices or click the download pdf button below. 

INSTRUCTIONS:

  1. Download the application form to your computer.
  2. Fill out the application form and save it.
  3. Email the .pdf to: infotcss@modocnation.com

**Signatures are not required upon submission of this form. We will require “Applicant and/or Custodial Parent” signatures at the first in-person appointment.
